Found in Intake Forms I hereby state that, to the best of my knowledge, my answers to the above questions [found in your intake forms] are correct.I agree and consent to assessment and treatment. I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment.Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage.
General Liability Release
I Therefore Release the Company and the Individual massage therapist from ALL liability concerning these injuries that may occur during the massage session.
Inappropriate Behavior Policy
Massage Therapy/Foot Zone is for relaxation and therapeutic purposes only. THERE IS ABSOLUTELY NO SEXUAL COMPONENT TO MASSAGE WHATSOEVER. Any insinuation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and a refusal of ANY and ALL services in the future. You will be charged the full service fee regardless of the length of your session. Depending on the behavior exhibited we may also file a report with the local authorities if necessary. Treat your therapist with respect and dignity and you will be treated the same in return.
We respectfully ask that you provide us with a 48 hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing more than a 48 hour notice we are often unable to fill that appointment time. This is a LOSS OF INCOME to your therapist and also means our other clients who are serious about receiving massage treatments miss the chance to receive the services they need. For this reason, If you cancel within the 48-hours before your appointment you are asked to pay the 100% of service fee. (Example: if your appointment is on Wednesday at 9:00 AM, and you call Monday at 12:00 PM to cancel, then you are expected to pay 100% of the service fee.)
No-Show No Call
Not showing up for an appointment without proper notification will result in an AUTOMATIC 100% of service fee.
We reserve the right to require a credit card number to be given to book appointments so that appropriate fees may be charged if a late cancellation does occur. We reserve the right to charge your card placed on file to uphold this policy.
We understand that emergencies can arise and illness do occur at inopportune times. IF you have a FEVER, a known infection, or have experienced vomiting or diarrhea within 24 hours prior to your appointment time, we REQUEST THAT YOU CANCEL your session. Inclement weather may also result in the need for cancellations. We will do our best to give advanced notice if we are closing or need to cancel due to bad weather and we ask you to do the same. Please DO NOT risk your own safety trying to make your appointment. Late cancellations due to emergency, illness, or inclement weather will generally not result in any missed session charges but this is determined on a case-by-case basis.
Example of valid excuses: "My kid is injured/sick and needs to go to the Hospital/Urgent Care"
"My relative/friend pasted away today" or "the funeral is today", "I woke up and have a fever/flu like symptoms/ or threw up"
Examples of NOT VALID excuses with in the 48-hour and day of window: "I got called into work"; "I forgot to reschedule because work changed my shift"; "We are not going to be coming into town today"; "I forgot something else came up"; or "I forgot".
Great, glad you can pay your bills how will your massage therapist?)
We request that you arrive 5-10 minutes prior to your appointment time to allow time to fill out any required paperwork as well as answer any intake question your therapist may have. We understand that issues can arise that may cause you to be late for your appointment. However, we ask that you call to inform us if this ever occurs so we can do our best to accommodate you. Appointment times are reserved for each client. So oftentimes we cannot exceed that reserved time without making the next client late. For this reason, arriving after you appointment time may result in loss of time from you massage/Foot zone so that your session ends at the scheduled time. Full service fees will be charged even when sessions are shortened due to late arrival. In return we will do our best to be on time, and if we are unable to do so we will add time to your session to make up for OUR late arrival or adjust the service charge accordingly.

Massage & COVID-19
Some questions you may have regarding your upcoming massage appointments or future scheduling.
How do you sanitize?
We have always cleaned the room and sanitized between appointments, and will continue to do so. By cleaning any and all tools used during the appointment with hot water, soap and rubbing alcohol, changing sheets, airing out room between appointments with open door, and sanitizing surface areas with Lysol.
Do you treat clients who are sick?
NO, if you are ever sick in ANY way, DO NOT come in. We have always required this of clients and our Massage Therapists to reduce the spread of ANY sickness.
What are your COVID Prescreen procedures?
During the worst of the pandemic we required clients to prescreen before every appointment which included stating if they have any symptoms or been around people with possible symptoms etc. Now it is assumed that when you confirm your appointment that you are not sick and will not be a risk to our massage therapist and other clients.
Do you require masks to be worn?
Our clients' health is of the utmost priority. We follow CDC guidelines- those who have been vaccinated do not have to wear a mask. We expect clients to use their best judgement for their health. For clients who decide to wear a mask, they are informed of the dangers of wearing a mask while face down. This is to say that when you lay on your stomach it applies pressure to the lungs and reduces the amount of oxygen intake, couple that with the pressure added during a massage and the reduction of oxygen that masks create which can lead clients to passing out. The real danger is in that the massage therapist will not fully be able to tell the difference if you are asleep or past out.
Does massage help overcome sickness?
NO, there are several contraindications of receiving massages while sick. Massage activates increased blood flow and helps to drain lymph because of this it can make you even more sick. By draining the lymph into a faster blood flow, the virus can travel faster than if you let your body do the work at rest. Thus, you can feel more sick.
BUT massage is beneficial after you have healed from the sickness and are no longer contagious. Although again because we help the lymphatic system drain any lingering by-products in the lymph, this can give you the symptoms you had when sick.
What if it I think it is just allergies?
We trust clients to know and be honest if they are actually having seasonal allergies verses being sick and will work on them.
Can essential oils benefit sickness?
Essential oils do not claim to cure all or be completely full proof. Everyone reacts differently to their use. If requested the use of essential oils can be applied during your massage.
What are the risks?
Just like any other sickness, the common cold etc. We can not guarantee that you will not come into contact with these viruses/diseases/ etc. That is a risk you accept every time you exit your home. We will do our best to keep things clean and sanitized as always. You accept this risk when you schedule and keep your massage appointments.
What do I expect from my clients?
We ask each of you to be respectful of those around you. If you think you are coming down with something, please STAY HOME. If you think you will be putting others at risk please stay home.
