Found in Intake Forms I hereby state that, to the best of my knowledge, my answers to the above questions [found in your intake forms] are correct.I agree and consent to assessment and treatment. I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment.Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage.
General Liability Release
I Therefore Release the Company and the Individual massage therapist from ALL liability concerning these injuries that may occur during the massage session.
Inappropriate Behavior Policy
Massage Therapy/Foot Zone is for relaxation and therapeutic purposes only. THERE IS ABSOLUTELY NO SEXUAL COMPONENT TO MASSAGE WHATSOEVER. Any insinuation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and a refusal of ANY and ALL services in the future. You will be charged the full service fee regardless of the length of your session. Depending on the behavior exhibited we may also file a report with the local authorities if necessary. Treat your therapist with respect and dignity and you will be treated the same in return.
We respectfully ask that you provide us with a 24 hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24 hour notice we are often unable to fill that appointment time. This is an inconvenience to your therapist and also means our other clients miss the chance to receive the services they need. For this reason, you will be charged 50% of the service fee for the first missed session and 100% of the service fee for each session after that. We also reserve the right to require a credit card number to be given to book future appointments so that appropriate fees may be charged if a late cancellation does occur.
We understand that emergencies can arise and illness do occur at inopportune times. IF you have a FEVER, a known infection, or have experienced vomiting or diarrhea withing 24 hours prior to your appointment time, we REQUEST THAT YOU CANCEL your session. Inclement weather may also result in the need for cancellations. We will do our best to give advanced notice if we are closing or need to cancel due to bad weather and we ask you to do the same. Please DO NOT risk your own safety trying to make your appointment,. Late cancellations due to emergency, illness, or inclement weather will generally not result in any missed session charges but this is determined on a case-by-case basis.
We request that you arrive 5-10 minutes prior to your appointment time to allow time to fill out any required paperwork as well as answer any intake question your therapist may have. We understand that issues can arise that may cause you to be late for your appointment. However, we ask that you call to inform us if this ever occurs so we can do our best to accommodate you. Appointment times are reserved for each client. So oftentimes we cannot exceed that reserved time without making the next client late. For this reason, arriving after you appointment time may result in loss of time from you massage/Foot zone so that your session ends at the scheduled time. Full service fees will be charged even when sessions are shortened due to late arrival. In return we will do our best to be on time, and if we are unable to do so we will add time to your session to make up for OUR late arrival or adjust the service charge accordingly.
BY SIGNING BELOW, [in your intake forms] you agree that you have read the above statements, procedures, liabilities, and policies. You also agree to abide by the above policies.
Massage & COVID-19
Some questions you may have regarding your upcoming massage appointments or future scheduling.
How do you sanitize?
I have always cleaned the room and sanitized between appointments, and will continue to do so. By cleaning any and all tools used during the appointment with hot water, soap and rubbing alcohol, changing sheets, airing out room between appointments with open door, and sanitizing surface areas with Lysol.
Do you treat clients who are sick?
NO, if you are ever sick in ANY way, DO NOT come in. I have always required this of clients and myself to reduce the spread of ANY sickness.
What are your COVID Prescreen procedures?
I have alway preformed the cleaning guidelines stated by the State in regards to equipment and myself. I also require clients to prescreen before every appointment which includes stating if they have any symptoms or been around people with possible symptoms etc. If you wish to see my prescreen from please email me.
Do you require masks to be worn?
My clients' health is of my utmost priority. I do what I can to follow state guidelines. I also expect my clients to decide for themselves what is best for their health. I personally have some medical issues that make it extremely difficult to wear a mask while I work (I will share my medical problem if asked directly). I do make a more concerted effort to not breath on my clients, which again has always been a procedure of mine. If you are a high risk client who requests my use of a mask I inform them that my body can handle about 20 minutes of mask time and will only wear one while they are face up. For clients who decide to wear a mask, they are informed of the dangers of wearing a mask while face down. This is to say that when you lay on your stomach it applies pressure to the lungs and reduces the amount of oxygen intake, couple that with the pressure added during a massage and the reduction of oxygen masks create which can lead you to pass out. The real danger is in that I will not fully be able to tell if you are asleep or past out.
Does massage help overcome sickness?
NO, there are several contraindications of receiving massages while sick. Massage activates increased blood flow and helps to drain lymph because of this it can make you even more sick. By draining the lymph into a faster blood flow, the virus can travel faster than if you let your body do the work at rest. Thus, you can feel more sick.
BUT massage is beneficial after you have healed from the sickness and are no longer contagious. Although again because we help the lymphatic system drain any lingering by-products in the lymph, this can give you the symptoms you had when sick.
What if it I think it is just allergies?
Usually, I trust clients to know and be honest if they are actually having seasonal allergies verses being sick and will work on them. BUT, because of the minimal symptoms of COVID-19 seen in healthy people you could be a carrier of COVID-19 and not know. PLEASE stay home. Otherwise, you will put people at risk.
Can essential oils benefit sickness?
Essential oils do not claim to cure all or be completely full proof. Everyone reacts differently to their use. IF requested I can use my immune boosting essential oils during your massage.
Can you do in home massages?
Yes, if you do not want to venture out of your home, I do offer in home massage for an additional $20. I will allow the use of gift certificates and packages to be used at this time for in home massage with the additional set up fee of $20. (I reserve the right to change this offer at any time)
BUT if you are sick or any member of your family is sick - I CAN NOT provide this service to you. As a courtesy I will also keep you informed if I have become exposed to the virus or had lots of contact with people.
What are the risks?
Just like any other sickness, the common cold etc. I can not guarantee that you will not come into contact with these viruses/diseases/ etc. That is a risk you accept every time you exit your home. I will do my best to keep things clean and sanitized as I always do, but there is still a risk because I cannot control other people’s actions. You accept this risk when you schedule and keep your massage appointments.
What are you (Jessica, LMT) doing to prevent the spread of COVID-19?
I am doing everything I can to stay informed and ahead of this pandemic. I am limiting the amount of time I spend in largely populated public areas. I continue to wash my hands and sanitize my home and studio space. I have asked those who may be at risk or put others at risk to stay home. I avoid those who I know are sick (with or without COVID-19). I will also be limiting the amount of massages I will do each day to reduce the amount of traffic in my studio.
What do I expect from my clients?
I ask each of you to be respectful of those around you. If you have high contact with masses of people please consider rescheduling. If you think you are coming down with something, please STAY HOME. If you think you will be putting others at risk please stay home.
